You need to know the following to pre admit at the hospital before your date of surgery.

• We will provide forms with all the procedure codes, ICD10 codes, doctors practice nr and the hospitals practice nr (see example below)
• If your surgery is on a Monday, Tuesday or Thursday, you need to go to Sr Marina Nel, tel: 011 724 2086, at pre admissions (next to reception, opposite the coffee shop) at the Union hospital to book your bed for surgery.
• If your surgery is on a Friday you need to go to Sr Susan Joubert, tel: 011 724 2351, at pre admissions (opposite the coffee shop) at the Clinton hospital to book your bed for surgery.
• If your surgery is on a Monday or a Tuesday, you must report at the Union hospital’s reception on the day of surgery, no later than 06:15.
• If your surgery is on a Thursday, you must report at the Union hospital’s reception on the day of surgery, neither earlier nor later than 12:15.
• If your surgery is on a Friday, you must report at the Clinton hospital’s reception on the day of surgery no later than 07:30.

NB: It is YOUR responsibility to get authorisation from your medical aid!
